ABSTRACT

We report the case of a 27-year-old male patient with dyspnea on physical exertion. Clinical assessment and various tests led to the diagnosis of aortopulmonary window and double aortic arch. According to a literature search, this may be the first report on such association

ABSTRACT

Objetivo Avaliar aspectos angiográficos do coração univentricular. Métodos Trinta e um pacientes divididos, de acordo com o tipo morfológico de coração univentricular, em tipos ventricular esquerdo, direito e indeterminado. Analisaram-se a conexão átrioventricular, a posição de câmara rudimentar em relação à principal, a conexão ventrículo-arterial, a relação das grandes artérias entre si e, quando possível, a origem das artérias coronárias. Resultados Ocorreram 16 casos do tipo ventricular esquerdo com presença de conexão A-V tipo 2 valvas em 8 e tipo valva única em 6. A câmara rudimentar foi superior e à direita em 4 casos de dextrocardia, e superior e à esquerda com 8 casos de levocardia. A conexão ventrículo-arterial foi discordante em 9 pacientes, concordante em 4, dupla via de saída de câmara rudimentar em 2 casos e saída única de câmara rudimentar em um caso. A aorta era anterior a pulmonar em 13 pacientes. De 9 casos estudados, as artérias coronárias nasciam anomalamente em 5. Tipo ventricular direito ocorreu em 9 pacientes e havia ausência de uma das conexões A-V em 5. Bolsa trabecular foi identificada em 6 casos, sempre em posição póstero-inferior. Conexão ventrículo-arterial foi tipo dupla via de saída da câmara principal em 8. A aorta era anterior em 66,6%. Tipo indeterminado ocorreu em 6 pacientes, com conexão A-V através de valva única em 5. Em 3 havia dupla via de saída da câmara única com estenose pulmonar e nos restantes havia atresia pulmonar...


Purpose Evaluation of angiographic features of univentricular heart. Methods Thirty-one patients were divided into 3 groups according to morphological type of univentricular heart: left ventricle type, right ventricle type and indetermined. They were studied for atrio-ventricular junction, position of rudimentary chamber in relation to principal chamber, ventricle-arterial junction, the position of the two great arteries in relation to each other and whenever possible, the origin of the coronary arteries. Results There were 16 cases of left ventricle type with A-V junction through 2 valves in 8 and through single valve in 6. The rudimentary chamber position was superior and to the right in 4 cases of dextrocardia and superior and to the left in 8 of levocardia. The ventricle-arterial junction was discordant in 9 patients and concordant in 4, double outlet of rudimentary chamber in 2 and single outlet in 1. The aorta was anterior to the pulmonary artery in 13 patients. Anomalous origin of the coronary arteries occurred in 5 of 9 cases studied. Right ventricle type occurred in 9 patients with absence of one of the A-V junctions in 5. Trabecular pouch was seen in 6 patients, posterior and inferior positioned in all of them. Double outlet from principal chamber occurred in 8 patients. The aorta was anterior in 66.6%. Indetermined type occurred in 6 patients with A-V junction through single valve in 5. In 3 there was double outlet from principal chamber showing pulmonary stenosis and in the remaining 3 there was pulmonary atresia...
